REPORTS TO: Director of Nursing or RN Supervisor

WAGE: Hourly

WORK HOURS: Day Shift/Evening Shift/ Night Shift/Weekends/Variable

WORK ENVIRONMENT: 95% Indoors, 5% Outdoors

WORK PACE: Self paced. Ability to make decisions quickly and accurately in a fast-paced and emotionally demanding work area.

JOB RESPONSIBILITIES: Responsible for the delivery of quality total resident care on assigned unit, floor or wing.


Provides quality total resident care

1. Assesses resident’s needs

2. Defines resident’s problem areas and strengths

3. Develops plan of care

4. Implements plans of care

5. Evaluates resident’s responses to plan of care

6. Observes for and makes assessments when there are changes in residents’ condition

7. Reports to the physician as necessary – receives, transcribes and carries out orders

8. Does necessary documentation in regard to all of the above (1-7)

9. Cooperates and communicates with other disciplines

10. Follows established facility policies and professional standards in giving care

Maintain resident care standards

1. Investigates, takes action necessary, documents and reports all accidental and negligent occurrences related to residents, staff or others in the building

2. Identifies needs and secures supplies and equipment to provide resident care

3. Reports all problems with equipment or supplies to nursing office immediately

4. Monitors use of supplies to insure appropriate and judicious use by staff

5. Understands and satisfies State and Federal regulations establishing minimums for resident care

6. Utilizes appropriate procedures, makes suggestions for improved facility operation and resident care

Exercises supervisory responsibilities over non-supervisory team members

1. Interviews and makes final decision on whether to hire job applicants

2. Properly enforces facilities disciplinary policies through preparation and issuance of disciplinary notices. Decides and implements disciplinary suspensions and discharges

3. Maintains a good work environment by effectively adjusting employee grievances

4. Prepares, signs and issues performance evaluations

5. Assigns employees to work according to the facilities personnel policies and procedures, and in view of employee’s skill, ability and experience

6. Determines which employees will be laid off and recalled under the facilities personnel policies

7. Approves or denies requests for leaves of absence and vacation

8. Assigns overtime

9. Conducts meetings with subordinate team members at the beginning and end of every shift

10. Supervises team members in basic nursing procedures

11. Participates in ongoing orientation and on the unit in-services for team members

12. Attends regular staff meetings; contributes appropriate information and suggestions

13. Contributes to facilities positive employee relations environment by expressing appreciation of employees, by treating them with dignity and respect, and by answering questions or resolving complaints whenever possible

Participates in maintenance of adequate resident care

1. Utilizes all supplies and equipment appropriately and judiciously

2. Reports all problems with supplies or equipment promptly

Participates in opportunities for professional growth

1. Participates on facility committees such as infection control, safety, etc. as opportunity arises

2. Attends facility in-services and seminars

3. Attends outside continuing education or formal education or formal education programs related to health care

4. Belongs to professional and/or community organizations related to health care

Promotes and maintains safe work environment

1. Gives frequent safety instruction

2. Investigates thoroughly both accidents and near-accidents

3. Identifies and corrects unsafe acts and hazards

4. Obtains accident-free work from subordinates

5. Performs accident-free work

6. Abides by all safety rules

7. Complies with procedures for safe lifting, ambulation, and transfers of residents

8. Able to perform and carry out Disaster Plan appropriately

9. Attends mandatory in-services regarding safety

Performs other duties as assigned by supervisor

Salary: $22.00 /hour
