POSITION: Clinical Support Specialist

Summary of Role: The Clinical Support Specialist will travel to communities in our portfolio needing nurse leadership. The role will be responsible for filling in where the Health and Wellness Director role is vacant and/or assigned to areas that need clinical leadership and support. You will provide leadership over the physical, mental and social health of our residents. In this role, you will build a caring relationship with residents and ensure their medical care plan is applied. You will have the opportunity to know the residents and their families to guarantee open communication and promote a sense of well-being.
Duties and Responsibilities of the Position:
- This is a traveling position and will require the candidate to travel to any of our communities, across 8+ states.
- Expected 90% travel depending on home base.
- Manage the coordination of care for residents, including the dissemination of information to families, staff, physicians and third-party providers.
- Perform pre-admission and ongoing assessments of residents’ physical and psycho-social needs.
- Order medications and oversee the resident’s medication schedule.
- Assure that all medications prescribed are available by auditing the medication records and cart twice weekly and ensure proper documentation for medication assistance administration.
- Provide training, supervision, and monitoring of Medication Aides and/or staff nurses in the administration and documentation of medications.
- Write and review residents’ assessments/evaluations and care/service-plans with appropriate staff.
- Work with the residents, families, and other staff to ensure that residents are at appropriate levels of care.
Pay: From $80,000.00 per year
