Rapidly growing medical practice providing Chronic and Transitional Care Management for patients in the SNF, LTC, ALF, and ILF settings seeking LPN with experience in at least one of these settings to work as a Care Management Coordinator and Triage Nurse. The right candidate is a highly motivated, efficient, task-oriented individual who can effectively manage multiple telephones, text, and emails quickly and professionally. This position requires a high level of discipline and autonomy. This position primarily works from home but does require monthly meeting attendance. Reliable transportation and a current Colorado LPN license required. Must be located in the greater Denver area.
Coordinate chronic and transitional care management with patients, family members and provider teams. Receive incoming calls and faxes from facilities, pharmacies, hospitals and patients; contact our providers spread throughout the greater Denver area and beyond, and quickly respond back to nursing or administration at the facilities with orders
Generate individualized care plans for patients enrolled in chronic care management
Obtain and review discharge summaries for transitional care
Schedule face-to-face visits for patients with providers
Travel to patient homes to assist with TCM face to face encounters
Answer and return calls from patients and family members
Document all patient and nursing communication in EMR/EHR as directed
Track metrics, communication, and projects as assigned
Triage all incoming calls, faxes and emails accordingly on assigned Triage Nurse days for cross coverage
Participate in regular monthly meetings and assist with set up and clean-up of meeting areas
Proactively seek ways to improve workflow, patient care, and communication for the Triage Team as well as the entire Practice.
Understand the prioritization of tasks and be able to make appropriate judgment calls regarding daily use of time
Ability to adapt to change and new expectations as workflows, tech systems, etc.
Respond timely and professionally to all items within your scope of practice as an LPN
Other duties as assigned
Minimum 1-year experience as a nurse at a SNF, LTC, ALF, or ILF
Minimum 1-year experience as Care Coordinator
Current LPN license in Colorado
Minimum 1-year experience with documentation in an Electronic Health Record
