KHP is committed to providing high-quality, patient-centered care. We are seeking a compassionate and skilled Family Nurse Practitioner to deliver primary care and health promotion services to our diverse patient population.
Position Summary: Under the direction of professional medical providers, the Family Nurse Practitioner will perform comprehensive primary care services. The ideal candidate will provide quality, cost-effective care, ensuring patient confidentiality and maintaining timely documentation in accordance with clinic protocols.
Key Responsibilities:
Health Promotion Services:

Conduct school physicals, immunization clinics, well-child and well-woman clinics, adult focus clinics, blood pressure screening clinics, nutritional counseling, and family planning counseling.

Provide prenatal care when applicable.
Patient Care:

Perform, order, and interpret laboratory tests, x-rays, and other diagnostic tests.

Obtain and record patient histories and physical examinations.

Evaluate and treat minor illnesses and injuries.

Monitor and treat chronic diseases.

Perform surgical and other medical procedures within competency.
Health Education:

Conduct health education activities for individuals and groups.

Promote health by advising patients on diet, hygiene, and disease prevention methods.
Consultation and Referrals:

Obtain consultation or referrals for services not provided by the clinic.

Collaborate with healthcare professionals to ensure comprehensive care.
Additional Duties:

Provide additional services as agreed upon with the Medical Director and Executive Director.

Participate in quality improvement activities.

Qualifications:

Completion of MSN and Family Nurse Practitioner program.

Current, valid license to practice medicine in the State of Oregon, including DEA license and CPR certification.

Current BLS certificate.

Relevant and substantial knowledge of health center licensing and operations, particularly within an FQHC environment.

Ability to work effectively both independently and as part of a team.

Commitment to maintaining the highest level of confidentiality.

Sensitivity to the needs of a culturally diverse patient and employee population.

Willingness and ability to work some evenings and weekends as needed.

Physical Demands and Accommodations:

Must be able to lift and/or move up to 25 pounds occasionally.

Work Monday through Friday, 8:00 a.m. – 5:00 p.m., with occasional extended hours and Saturdays.
