Covenant Health is seeking a Full Time Evening Shift Social Worker for the Psych Acute-Adult CCH Department at Covenant Children's Hospital.
Provides education and support services to patients, families and staff. Provides psychosocial assessment, counseling, crisis intervention, referrals and information to patients and families. As a complex Discharge Planner and a member of the Case Management Department, works collaboratively with the interdisciplinary team to effect positive patient and family outcomes. Serves as resource for less-experienced social workers.
Based on referral from Case Manager, Social Worker or other member of the healthcare team (i.e. physician, RN), completes comprehensive psychosocial assessments to evaluate the patient/family to identify ability to meet patient care needs.
Based on referral from Case Manager, Social Worker or other member of the healthcare team (i.e. physician RN), provides counseling/support/ education to patient/family. Assists patient/family in coping with hospitalization, disability, and chronic/terminal illness.
Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of patient illness. Ensures integration of the interdisciplinary team in understanding and integrating these aspects into the plan of care.
Provides complex counseling for patients and family members as appropriate when counseling needs are identified.
Adheres to the regulatory requirements for social work intervention/oversight based on state and federal guidelines (i.e.: ESRD, transplant and rehabilitation patients).
Provides crisis intervention, supportive counseling and initiates referrals to appropriate resources to ensure patient/family psychosocial and emotional needs are addressed.
Maintains current knowledge and researches availability of community agencies and community resources for social, emotional or financial assistance.
In collaboration with the Case Manager, provides information, education to patient/family on community resources and options for post-acute care appropriate to the age of the patient served. Initiates referrals to community agencies as needed.
Facilitates discharge arrangements in collaboration with patient/family and the healthcare team ensuring arrangements meet patient continuing care needs. Ensures services are arranged and communicated. As needed, follows up internally to ensure services are provided as arranged.
Understands placement intricacies and can interpret requirements from state, local, and federal agencies to optimize placement of patients in the most appropriate setting Aligns needs of patient with appropriate placement options.
Identifies and records clinical, psychosocial and financial barriers to a smooth transition across the healthcare continuum and assists in identifying and facilitating system improvements.
Utilizes communication, negotiation, and advocacy skills with patient/family, staff, healthcare team, and community resources.
Maintains collaborative relationship with post-acute providers and community services.
Serves as a consultant for processing issues such as guardianship, abuse/neglect, power of attorney, healthcare surrogate, advanced directives, and psychiatric involuntary admission. Initiates/completes forms required for post-acute placement.
As patient is transferred, communicates pertinent information to next Social Worker and Care Manager to transition the patient to the next level of care.
Documents activities with entries that are consistent with meeting department/facility requirements, licensure and regulatory guidelines.
Supports interdisciplinary team conferences and contribute to the planning and review of care for patients presented as needed.
Adheres to standards, policies and procedures at both the Hospital and departmental level.
Promotes a service orientation in the performance of position duties and responsibilities and in interactions with patients, hospital staff and visitors.
Minimum Position Qualifications:
Masters degree in Social Work
Working knowledge/experience in managed care and payor issues
Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker in state of Texas (LCSW). Allowed to work with temporary licensure, but must obtain full licensure within 6 months of employment.
Adherence to the National Association of Social Work Code of Ethics
