JOB SUMMARY: The Nurse Home Visitor provides home visits to pregnant women and their families eligible for the NFP program. The Nurse Home Visitor is expected to adhere to the NFP model of home visitation, which includes: independent critical thinking within the nursing process; assessment of physical, emotional, social, behavioral, and environmental needs of women related to health and life course development, their unborn children, and their families; instruction/education and mentoring in the areas of prenatal and postpartum care, nutrition, parenting and family dynamics, child health care, family planning, and special health care needs; ; and facilitation of access to resources in the community to improve the outcome of the infant and the future of the client/family.This position will engage specifically with African American mothers and be part of the Healthy Families activities to reduce infant mortality and health disparities in Smith County. The maximum case load for each Nurse Home Visitor is 25 clients.
MINIMUM REQUIRED EDUCATION
- BSN required with a current Texas RN license in good standing is required.
- A minimum of 2 years recent experience in maternal/child health, public health, home visiting, or mental/behavioral nursing is required. Home visiting experience is preferred.
- Must possess excellent written and verbal communication skills, as well as basic computer skills. Valid driver’s license and an insured automobile are required. Must be able to work a flexible schedule, including some evenings and weekends as needed to meet the needs of the clients. Additionally, must be able to travel by air to Denver, CO for all face-to-face educational sessions and other off-site educational programs requiring maintenance of professional competence and compliance with NFP program requirements.
