JOB SUMMARY: The Mental Health Coordinator assists clients with obtaining, getting access to, and coordinating continued behavioral health treatment and social services. The Mental Health Coordinator also evaluates the need for customized services to the individual that will be delivered in the community as well as explores all available resources for the individual.
Essential duties and responsibilities include but are not limited to:
Mental Health Coordinator
- Accomplishes clients' goals and objectives by assessing needs and elaborating service plan; monitoring the client's mental condition, and linkage to community resources in order for the client to gain the needed tools to navigate the system and promote independence.
- Maintains clients' records by reviewing case notes and service plans.
- Monitors cases by verifying clients' attendance to providers; observing and evaluating treatments and responses; advocating for needed services in the community; obtaining additional resources
- Assists and supports the Case Managers.
- Provides services in compliance with contract and Agency regulations.
- Identifies service needs for individuals in the intake assessment and evaluation process, to include but not limited to the collection of demographic information as well as medical and clinical history
- Coordinates the development and implementation of the treatment plan with active participation from the treatment team, family, court and/or care giver by facilitating interagency team meetings and service planning
- Assists clients and their families in obtaining, accessing, and coordinating needed services in a timely manner, including but not limited to: case management, housing, recreational activities, specialized health and crisis services; educational, community and more intense programs, if required; advocacy.
- Performs other duties as assigned.
Education and Experience
- Bachelor's degree in psychology, sociology/social work or a related field from accredited institution OR at least three years of certified experience in social work, case management
- Three (3) years' experience working directly with individuals in behavioral health setting
- Previous case management experience preferred
- Demonstrated computer proficiency, especially with Microsoft Office Suite (Word), G Suite and Database
- Knowledge of community resources specific to the assigned region
- Experience working or volunteering with immigrants and refugees is a plus
Travel
Travel is primarily local during the business day with current valid driver’s license required.

- Fluency in English and Spanish (speaking, reading, writing)

- Knowledgeable about available resources for Latino immigrants;
- Uncompromising ability to maintain confidentiality.

While performing the duties of this job, the employee is regularly required to sit, to talk, to hear. The employee is occasionally required to stand, walk, and reach with hands and arms. This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, copiers, fax machines and filing cabinets.
