Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Housing Search Coordinator at our Worcester site.

The Rapid Re-Housing for Residential Recovery Services program was developed to meet the housing needs of persons graduating from Residential Recovery Services. The Rapid Re-Housing project model is designed to provide households access to flexible yet time limited funds that enable them to successfully maintain independent housing that supports their sustained recovery.

The Housing Search Coordinator demonstrates person centered strength based communication and relationships with Person’s and demonstrates effective verbal and written communication with staff and peers, supervisors, and other professionals.

Responsibilities
Work with Persons to identify housing needs and preferences.
Completes housing search within identified parameters for location, size, amenities and costs.
Accesses available funding for approved costs related to securing housing within established guideline.
Provides short-term case management of newly housed participants to support the maintenance of tenancy.
Maintains competencies: Human Rights, Dual Diagnosis, Infection Control, CPR, First Aid, Agency and Divisional Policy and Procedures as well as other required competencies.
Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

Qualifications
Grade 5: HSD/GED/HiSet required; Associates degree preferred
Grade 6: Bachelor's degree required; degree and/or coursework in social work, counseling, psychology or related field preferred.
2 years human service experience and 2 years supervisory experience.
Current valid US-issued drivers license and ability to provide registered, inspected and insured automobile for work related purpose, including transporting clients in your own vehicle
Must be able to pass a CORI background check
