*Overview*
First Choice Home Health and Hospice is a National Company with multiple location in the Bay area. We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide high-quality care to patients in the comfort of their homes, promoting wellness and independence.
*Duties*
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient vital signs and condition
- Educate patients and their families on disease management and treatment plans
- Collaborate with physicians and healthcare team members to ensure coordinated care
- Document patient care accurately and timely using electronic health record systems
*Skills*
- Proficiency in DME (Durable Medical Equipment) usage
- Experience with Kinnser Software
- Strong patient care skills with a focus on holistic wellness
-- Competency in IV, Woundcare and tube feeding procedures
Job Types: Full-time, Per diem
Pay: $60.63 - $66.81 per hour
