Merakey is looking for a Full-time, Hospital Liaison Case Manager to join our Harrisburg, PA Team.

POSITION REQUIREMENTS

  • Bachelors' Degree in Human Service field with 2-3 years of experience
  • Or a Masters' Degree in Human Service Field with 1 year of experience.
  • Masters Preferred.
  • Must have valid Driver's License and access to reliable vehicle during business hours.

POSITION OVERVIEW

  • Collaborate with inpatient hospitals to facilitate discharge plans to Merakey Services for new consumers and existing consumers with Merakey.
  • Engage the individual and ensure understanding of services. Assist with development of their discharge plan and provide transportation to appointments after discharge.
  • Coordinate referrals to Merakey services and assist with referral process, intake needs and any process that needs completed for services.
  • Collaborate with other agency supports and case managers to ensure all needs are being met.
  • Ensure that individuals have transportation to appointments after discharge and bridge them with resources.
  • Participate in Quality Improvement activities and collaborate with Merakey programs to coordinate services for individuals.
  • Lead Team Meetings for individuals being discharged from the hospital to ensure all needs are met at discharge. Include all providers of services in meetings as needed.
  • Assist with completing assessments and documentation in programs for new or existing Merakey consumers.
  • Coordinate Physical healthcare follow up as needed and refer to appropriate services.
  • Provide marketing materials about Merakey programs to hospitals and assist with any questions about services.
  • Develop working relationships with Healthcare providers in the area to establish resources
  • Attend all trainings and meetings as assigned
  • All other duties as assigned
