As a Licensed Practical Nurse (LPN) at an Intermediate Care Facility/IID, the staff person is responsible for the biological, psychological and social assessment of clients. The staff person assists with the coordination, facilitation and implementation of health care objectives of the Individual Program Plan (IPP), including coordination of medical, dental and psychological evaluations. The staff member collaborates with members of the Interdisciplinary Team (IDT) and other health care professionals in the community to ensure all of the individuals needs are being met in a timely manner. Within the scope of practice the staff member provides technical support to non-licensed staff regarding health care services and medication administration. At the direction of the Registered Nurse (RN) the LPN, within the scope of their practice, will assess, implement and evaluate the individual’s healthcare needs for all acute and chronic healthcare conditions. The staff member will perform on going quality assurance of all individual information, medications, treatments and devices used or needed for the individuals residing in the ICF. The staff member may be required to participate in training, meetings and participate in community engagements to further support the individuals residing in the ICF. The staff member will communicate in an effective, respectful, professional manner using methods easily understood by all parties involved. The staff member may be required to work evening, weekends, overnights and/or holidays to ensure adequate nursing coverage to the Developmental Services Division.

This is an Alpha 1 position - in the event of inclement weather or other emergencies, you may be required to report to your work site.

Minimum: Possession of a valid license issued by the State of Virginia as a Licensed Practical Nurse.
Must have, or obtain, a current and valid Virginia or North Carolina driver's license in accordance with Virginia or North Carolina DMV driver's license eligibility requirements.

Requirements: All employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies.

Experience working directly with the ID/DD population
Experience in nursing assessment and documentation
Experience with managing Gastrostomies and Jejunostomies
