General Summary of Position:
The Community Health Outreach Manager oversees community benefit/community health activities and identifies, develops and/or sustains working relationships with public, private and faith-based partners. The outreach manager will identify, test and implement new programs and services that strengthen the relationship between the hospital and the community. Special emphasis will be placed on cultivating community-based interventions that optimize the health and well-being of discharged patients. The manager also establishes and manages institutional procedures and practices to capture, document, validate and report community benefit activities in accordance with IRS guidelines. He/she oversees all administrative duties for the Community Relations Department.

Education:
Master’s degree in religious studies, public health, nursing, or related field is required. Knowledge of healthcare and/or wellness theory, practice and administration to design, implement and oversee community health services for vulnerable populations. Grant writing experience is strongly desired.

Experience:
Approximately two to three years of progressively more responsible job-related experience.

Certification/Registration/Licensure:
Certification as a Community Health Education Specialist (CHES) is preferred.
