A Primary Care Physician is sought for Community LIFE, a Program of All-Inclusive Care for the Elderly (PACE) at our brand new Bedford, PA location. Our PCP is responsible for managing the medical conditions of our frail, elderly members, and for oversight of their use of medical specialists and inpatient care.
The Physician is a member of the Interdisciplinary Team (IDT), which includes a Registered Nurse, Social Worker, Registered Dietitian, Physical and Occupational Therapists, Recreational Therapist, and Personal Care Aides. The IDT continually assesses each patient, then develops and provides a comprehensive (social, medical, spiritual) plan of care. The position requires an individual with a demonstrable grasp of preventive and palliative care, who can confidently lead and explain medical decision-making, while being a fair and helpful member of a larger group addressing a patient’s problems in a holistic manner.
The schedule for this position is flexible and can be adjusted to meet the needs of the physician. Community LIFE offers a good work/life balance with minimal on-call responsibilities.
Requirements include: MD degree from an accredited university. PA medical license, DEA Registration, Board eligible in Geriatric, Internal or Family Medicine, and a minimum of two years working as a physician, at least one year of which was with a frail or elderly population.
This position may be eligible for loan assistance under the Pennsylvania Primary Health Care Loan Repayment Program.
