Dynamic, innovative treatment team is seeking full-time employees to provide ongoing, intensive community psychiatric support services to clients who have severe mental illness, substance abuse issues, HIV, AIDS, and are homeless. The Case Manager will be responsible for assessment, mental health interventions and linkage using evidence based practice to assist clients in recovery. Formal training will be provided.
Qualifications: Qualified candidates must possess a Bachelor’s degree in social, behavioral or human services, coupled with at least one year of professionally supervised work in the delivery of mental health and/or chemical dependency services. A valid driver’s license with fewer than six points along with proof of insurance is required.
