Title: Care Coordinator
Department: Adult Day Services
FLSA Status: Non- Exempt
Reports To: Director
Summary: Responsible for coordinating the ADS processes and is responsible for all participant care related clerical procedures, coordination of services and processes and facilitates communication between participant/families and other team members.Provides oversight between professional staff, transportation, activities, program and rehab aides
1.Coordinates all daily participant and center operations including transit services of participants.
2.Overseas the operation of the group van program
3.Trains, supervises and evaluates office, kitchen and janitorial staff; trains and supervises volunteers for program compliance
4.Coordinates on-call staff schedule to ensure coverage for the program
5.Maintains inventory of medical and food service supplies and replenishes first aid and emergency supplies regularly
6.Liaises with vendors/contractors and maintains up-to-date contracts
7.Maintains records of daily attendance of participants to generate statistical reports for management and funders
8.Prepares list of participants for quarterly and 6-month reassessments
1.Certified Nursing Assistant, high school graduate, preferably an associate degree.
2.At least three (3) years’ experience in health care office administration and care coordination
3.Demonstrated ability to supervise administrative personnel and program aides
4.Great attention to detail, good organizational skills and ability to manage and prioritize multiple tasks
5.Proficient in MS Office, word processing and electronic health records
6.Demonstrates good communications skills, negotiation skills, and public relations skills.
7.Bilingual in English and Chinese
8.Must have TB clearance prior to employment
