This position is located at 2100 Charlie Hall Boulevard, Charleston, SC 29414.
Under general supervision, provides assessments for patients being discharged from local hospitals, consultation, crisis intervention and short term case management services. The overall purpose of this position is to assure timely access to outpatient mental health services for applying consumers and to divert future hospitalization.
A bachelor's degree and professional experience in human services or social services program (s).
A Master's or Doctorate degree from an accredited program, university, or college in psychology, social work, counseling, marriage and family counseling or other behavioral discipline deemed equivalent by SCDMH and two (2) years of post-graduate experience in a clinical counseling program.
