The Schedulers primary function is to facilitate scheduling of home visits for the field staff each day and assisting with the coordination of patient care. Ensuring geographic scheduling to reduce drive time and to enhance staff satisfaction and patient care.
- Schedules home visits for the field staff each day and assists with the coordination of patient care.
- Manages clinician tracker to ensure visits are allocated appropriately across all RNs, LVNs, and Therapists. Monitors and tracks staff productivity using current metric system.
- Creates and maintains staff schedules for Nursing, Therapy and Home Health Aide services. Enters schedule changes in staff schedules and reassigns patients appropriately.
- Ensures geographic scheduling to reduce drive time and to enhance staff satisfaction and patient care. Schedules all new referrals to the appropriate clinician based upon location of the patient and clinician availability.
- Verifies with authorization department available number of visits prior to scheduling.
- Contacts clinicians and clients regarding day-to-day changes, responsible for assisting field staff on calling for scheduling if unable to reach patient.
- Assigns all disciplines and clinicians in the electronic scheduling system.
- Provides staffing for sick leave, vacation, long term leave.
- Maintains call – in reports.
- Ensure compliance with regulatory guidelines ex: 14 day supervisory visit, 14 -21day wound evaluation, 30 day therapy supervisory visit, 5 day recertification window.
- Participates in the continuous quality improvement processes related to the patient population.
- Other duties assigned by supervisor.
- Ensures all contents necessary and required maintained in the patient medical record and required items completed. i.e. consents completed, all documents dated, signed etc.
- Ensures that faxed items such as lab work, orders and supporting documents (H&P, discharge summary op reports) etc. are transitioned into the patient medical record and in their appropriate sections.
- Processes, maintains, sends and tracks physician orders. Ensures orders back signed within appropriate time frames. If needed interacts with physician offices either by phone or in person to obtain signed orders.
- Appraises supervisor of any delays and difficulty in obtaining orders so further delays are not experienced.
- Works with Compliance officer and Supervisor with any record requests from regulatory agencies, subpoenas or patient/families
- Provides back-up with answering phone calls and triaging to the appropriate individuals and/or departments.
POSITION QUALIFICATIONS
1. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements
and is in good working order.
