Healthly is recruiting physicians who are double board certified in Lifestyle Medicine and a primary care specialty. You will take care of a panel of Medicare and concierge patients, providing full spectrum primary care services, and helping to coordinate the Lifestyle Medicine team that includes health coaches, dietitians, personal trainers and social workers; and incorporates groups, ITLC’s, lifestyle medicine clinical protocols, and numerous other resources that allow lifestyle medicine to really happen.

Role Description
As a Primary Care Physician, you will care for a smaller panel of patients and oversee their health as a population. You will lead a team of allied health professionals and medical assistants. You will also work closely with other providers in daily, weekly and monthly huddles.

Santa Monica, California.
Responsibilities include:
- Conduct in-person or virtual visits typically in 30 minute appointment windows, with strong focus on guiding patients on developing their own plans for health
- Population health initiatives focused on proactive strategies to prevent problems before they arise
- Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation
- Understand and apply HCC documentation and ICD-10 coding
- Care coordination with other providers, specialists, testing facilities, agencies
- Phone triage and outreach
- Patient education
- Practice promotional activities, such as health seminars
- Home visits as needed
- Other duties as assigned
Language: Fluent Spanish is a strong plus.
Job type: Full time (Monday-Friday 8am to 5pm) or part time (minimum 3 days/week)
Compensation:
- $250,000 per year base salary (pro-rated if part-time)
- Opportunity for bonus of $50,000 to $100,000 per year based on patient panel size, clinical outcomes and quality performance
