  • Experience in mental health field and co-occurring/substance use preferred

Merakey is currently seeking a Behavioral Health Navigator to join our Behavioral Health Site Based Outpatient Program, at 2506 North Broad Street, Colmar, PA.

POSITION OVERVIEW

Responsibilities include but are not limited to the following:

  • The Behavioral Health Navigator brings to the team clinical expertise in the assessment, treatment and management of behavioral health issues while working with challenging medical diagnosis
  • Develops personal treatment plans, coordinating appointments, providing support, transportation
  • Educates individuals about mental health and physical diagnosis
  • Links with community resources to best support the individual
  • Is the liaison between health professionals and the individual
  • Assists with health insurance and paperwork
  • The home office location is Colmar Pennsylvania, but this is a community-based position
