Elevate Home Health is currently seeking to hire a part time Certified Home Health Aide (CHHA) / Home Health Aide (HHA) to be a part of our fast growing team!

Part time and prn opportunities are available for the general areas of: MONTEREY AND SALINAS.

POSITION SUMMARY: The Home Health Aide is expected to perform all of the duties related to a home health aide in a home environment, and requires the knowledge and ability to perform as a health care professional. Duties include care of patient’s ADL needs as well as other duties as assigned by the supervising clinician.

Receive and processes referrals for supervising clinician.
Observes and reports changes in client’s condition to supervising clinician.

Assists patients to improve in ADL’s (Activities of Daily Living).

Communicates patient’s supply needs to supervising clinician or medical staff.

Attends In-Service Ttaining programs to maintain currency in practice.
Maintains confidentiality and does not discuss patient or patient’s information outside of the agency.
Minimum Requirements
Must have a valid High School diploma or equivalent.

Minimum 1-year experience as a Certified Home Health Aide (CHHA).

Current certification as a Certified Home Health Aide (CHHA) by the state of California.
