Minimum Qualifications
Education/Training
Master's degree in Social Work, Psychology or related field.

Experience
1 year social work or related experience, preferably in a medical/health setting.

License/Certification/Registration

LCSW-C (Licensed Certified Social Worker - Clinical), Ph.D., or LCPC (LicensedClinical Professional Counselor) in the State of Maryland.

Knowledge, Skills & Abilities
Diagnostic and problem solving skills. Verbal and written communication skills. Basic computer skillspreferred.
Primary Duties and Responsibilities
Collaborates with and completes referrals to appropriate community agencies for assistance based on patient need. Initiates referrals in a timely manner. Uses responses to help patient resume life in the community and/or adjust to lifestyle changes.
Completes psychosocial history, psychiatric assessments or socioeconomic assessment as determined by health care team or high-risk indicators and documents in medical record and maintains statistical data per standard policy.
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
Coordinates inpatient admissions to UMH or makes referrals for care to other community agencies for patients who require psychiatric services. Coordinates the completion of requisite forms by doctors, patients, and patients' families for any services required.
Develops and maintains information on community resources and referral requirements based on patient population as identified in the unit scope of practice.
Facilitates communication between the patient and family and the healthcare team. Assesses and communicates the patient's social, cultural, emotional, and economic problems to medical staff and other healthcare providers as needed. Attends multidisciplinary discharge planning rounds.
Initiates and participates in multidisciplinary discharge planning rounds and collaborates with internal and external health care providers, patient and family to develop comprehensive discharge plans.
Organizes individual workload and sets appropriate priorities based on patient's medical plan, patients needs, and policy and procedures.
Orients and/or supervises students as assigned. Orients new staff as assigned.
Participates in multidisciplinary quality and service improvement teams, in meetings and on committees and represents the department and hospital in community outreach efforts.
Performs other duties as assigned.
Provides clinical assessment including diagnostic interviews for patients presenting in ER for psychiatric disorders or in crisis. Provides verification and pre-certification of insurance as needed and appropriately documents. Acts as a consultant to the ER personnel making recommendations for patient care. Provides documentation relevant to mental status exam as outlined in CIS policy and consistent with acceptable standards on all patients seen by CIS in the ER.
Provides coverage for CIS, PHP/IOP, OP, 6 West and UR activities as needed.
Provides crisis intervention and management when necessary. Provides therapeutic intervention with patients and family including individual marital, parental, family, and group therapies.
