The HMC Adult Medicine Clinic has an outstanding opportunity for a Community Health Navigator - Program Coordinator (Full-Time, Day Shifts). Community Health Navigators will be trained in motivational interviewing, navigating health care systems (Specifically Harborview Medical Center, UW) and in self-care for improving health and blood pressure control. Navigators will work within the Adult Medicine Clinic and report to Clinical staff. They will also work collaboratively with Health Educators from AARTH, and HMC providers in Interpreter services who have an established high functioning peer navigator program. Peer Navigators will conduct individual interviews with African American patients with high blood pressure to explore barriers and goals for health care, and will provide navigation for African American Patients with high blood pressure, the specifics of which will be determined by early feedback and community members
Job Duties & Performance Standards
Contributes to a participative and team-oriented environment.
- Attend relevant meetings related to department or hypertension control program
- Patient tracking for reporting purposes. Collect and submit daily activity logs/reports.
- Participate in the delivery of program survey and focus groups
- Establish and maintain record keeping/filing systems in support of individualized one-to-one services. Operate office equipment such as computer, copy/fax machines.
- Monitor the Clinic’s office supplies, including necessary forms.
- Possess in-depth knowledge of software systems including: Amion, scheduling software (EPIC), Microsoft Office products, and UW purchasing and Materials Management programs.
- Order, receive, and maintain inventory materials and supplies for blood pressure control program.
Coordinates and oversees system of processing referrals according to HMC and insurance guidelines and requirements.
- Communicate and coordinate with medical team/staff regarding patient and maintain documentation for Hypertension program reporting.
- Coordinate patient schedules based on needs and communicate with Manager, Departmental offices, and clinic Medical Director regarding scheduling program outcomes.
- Coordinate and oversee all aspects of Managed care/referral program
- Assist Manager to develop, review, implement and maintain policies and procedures directly related to provider scheduling, program grant requirements, and residency related issues.
- Prepare clinic, patient and/or physician schedules and assist with utilization/productivity reporting on a routine basis.
- Provides information directly to patients, staff, faculty, and residents.
Leadership / Decision Making support – Ability to assign, instruct and check in with clinic staff, health equity, and health educator partners
- Serve as a role model demonstrating resiliency in overcoming obstacles and managing their own health outcomes/blood pressure common to those with increased hypertension.
- Increase the awareness and knowledge of people of African American decent about health issues that disproportionately affect them.
- Promote responsible health practices among people of African American decent.
- Within appropriate limits, determines the best clinical provider staffing for the clinic
- Possesses in-depth knowledge of software systems including: Amion, scheduling software (EPIC), Microsoft Office products, and UW purchasing and Materials Management programs.
- Act as a resource person to providers regarding scheduling and call issues.
- Provide feedback to Manager regarding the clinic schedule if clinics are too busy or too slow
- Evaluate and actively problem-solve presenting situations that arise from providers who do not show up for work for any reason.
- Assist Manager to develop, review, implement and maintain policies and procedures directly related to provider scheduling, residency related issues.
- May need a Washington State Driver’s License
- May need to obtain and maintain a food Handlers Permit.
- Exceptional communication skills, written, oral, and telephone.
- Experience interacting and communicating with a diverse population, using sensitivity, tact, and discretion.
- Demonstrated computer experience and proficiency, preferably in a Windows-based environment.
- Demonstrated history of commitment to continuously improving quality and service.
- Maintains the highest standards of confidentiality, professionalism, and ethical conduct.
- Demonstrates independent judgment and decision making skills and appropriately prioritizes and handles multiple tasks.
High school graduation or equivalent, AND
Two years of experience in the program specialty, OR equivalent education/experience.
