Intensive Outpatient Program (IOP) Care Coordinator

Description
The Care Coordinator for IOP clinical programs functions to support patients in these programs through several roles. They will coordinate additional components of care including facilitating transfer to emergency or inpatient care when clinically indicated, in support to the clinical leadership of the program. This individual collaborates with all involved teams and partners to ensure that patient and family needs are met.
- Coordinating intake procedures into the programs.
- Serving as a co-facilitator in group therapy sessions.
- Coordinating communication with schools, parents, and other support systems in patients' lives.
- Coordinating discharge planning and post-program follow-up care.
- Conducts phone screenings and review completed assessment forms with all interest patients prior to admission to the program.
- Identifies and prioritizes clinical and psychological goals and problem solves to ensure follow-through on needed responses by multidisciplinary team.
- Ensures optimal patient coordination between IOP, any previous or post discharge providers, and schools with a focus on patient experience and outcomes.
- Completes psychological assessment, Plan of Action, and Plan of Action updates within established timeframe.
- Conduct Intensive Outpatient Treatment sessions. Conduct 3 sessions per week, 3 hours per session. Sessions must be in the afternoon or evenings.
Qualifications
- Must be 21+ years of age
- Bachelor's of Social Work required, with focus area in Addiction
- Master's of Social Work, with focus in Mental Health & Addiction preferred
- Experience in medical, mental health & addiction, or social services field is preferred
- Ability to communicate in English, both verbally and in writing
- Excellent communication skills
- Demonstrates interpersonal relationships in a manner which enhances communication, promotes conflict resolution and facilities staff development
- Basic computer knowledge
- Demonstrates leadership abilities

- Licensed Alcohol and Drug Counselor (LADC)
- Licensed Addiction Counselor (LAC)
- Alcohol and Drug Counselor Trainee (ADC-T)
- Licensed Social Worker (LSW)
- Licensed Graduate Social Worker (LGSW)
- Licensed Independent Clinical Social Worker (LICSW)
- Licensed Professional Counselor (LPC)
- Licensed Professional Clinical Counselor (LPCC)
- Personal experience receiving community services as a consumer, and/or experience with addiction
- Experience as an IOP facilitator
