About Brigham and Women’s Health Care Center - Westwood:
Opening October 1, 2018, the newly-constructed Brigham and Women’s Health Care Center-Westwood is looking for dynamic people with a flexible, collaborative spirit to be a part of a high-functioning, customer-focused team to serve the greater Westwood community! Our new Center is located at Westwood’s vibrant new University Station development of retail, restaurants, business, hotel, and health-related enterprises. While the Center’s location is over 15 miles from the main campus of BWH, it is an expectation that the Center’s services offered will comply with the standards of care found at our main campus. Physicians practicing at the Center will be credentialed through BWH and will provide the same standard of care that makes the BWH nationally recognized for clinical excellence. Staff are BWH employees and must demonstrate the same high level of competence and professionalism found at the main campus. Travel to main campus may be required for professional development or business reasons.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Accepts assignments for patient care within the scope of his/her preparation. The LPN is responsible for participating in assessment, planning and evaluation of ambulatory nursing care delivered, incorporating physical, emotional and socioeconomic needs of the patient.
Administers medication according to established policies and procedures. Documents observed patient reactions to medication therapy.
Demonstrates awareness of accepted standards of practice and participates in planning, implementation, evaluating and revising care as necessary. Accurately performs patient intake and patient/room preparation duties
Inform patients of laboratory and pathology results as directed by physician.
Responsible for procedure set-up and physician assistance for all procedures, using sterile technique as needed.
Performs triage functions as needed, including but not limited to; patient phone calls and patients who present to the Urgent Care Center.
Keep physicians, other staff, and patients up to date on waiting times as dictated by Urgent Care Center standards.
Assists with patient education including but not limited to; wound care, procedure preparation, disease management and medication instructions.
Documents interventions, teaching, prescription renewals and other nursing interventions in the appropriate IS/paper systems in accordance with BWH policies.
Follows infection control, regulatory, and compliance guidelines per hospital standards, including sterilization of instruments.
Makes appropriate use of supplies, equipment and other resources, helping to maintain a cost effective program.
Responsible for monitoring the inventory of equipment and supplies used for the delivery of clinical care, and assists the medical assistant with ordering and stocking as needed.
Consults with physicians, nurses, advance practice providers and other members of the health care team in the planning, delivery and evaluation of patient care.
Keeps informed of personnel policies and incorporate them into behaviors as indicated.
Attends all required staff meetings.
Performs annual competencies.
Performs other duties as assigned

Qualifications

QUALIFICATIONS:
Graduate of an LPN program with current Massachusetts Licensure as Licensed Practical Nurse.
Must be certified in Advanced Cardiac Life Support or willing to become certified upon hire

Willingness to cross-cover for other employees in other roles as the need arises.

Must be able to communicate with patients and other parties from a wide range of age groups, socioeconomic levels, cultures, languages, etc.
Must be able to assist movement impaired patients from waiting area to exam rooms in a respectful manner to the patients and their families, and that will not compromise treatment.
Ability to use personal computers and select software applications.

WORKING CONDITIONS:
Ambulatory clinic healthcare environment.
Some exposures to communicable diseases, unpleasant odors, needle and blood products in a normal patient environment

SUPERVISORY RESPONSIBILITY:
No responsibility for the work of others.

FISCAL RESPONSIBILITY:
No direct fiscal responsibility.

OTHER DUTIES AND RESPONSIBILITES:
Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by BWH/Partners.
Follows safe practices required for the position.
Complies with appropriate BWH and Partners policies and procedures.
Fulfills any training required by BWH and/or Partners, as appropriate.
Brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital staff.
