Residential Nurse, RTC

The Residential Nurse provides direct nursing care to residents, such as administering and ordering medication, conducting medical assessments and diagnostic testing, and medical treatment as prescribed by a licensed medical or clinical professional in accordance with Agency policies, licensing and funding standards, and other federal, state, and local regulatory requirements.

Essential Duties

  • Provide resident with routine health screenings and perform basic health assessment; assess each resident’s unique medical needs and ensure they are properly met in a timely manner; report routine medical needs to the medical and clinical staff per Agency policy and licensing standards
  • Administer medication and treatment to resident as prescribed or as needed in accordance with licensing standards and nursing practices; coordinate medical appointments and follow through on medications and treatment as prescribed; coordinate and oversee the prescribed nutritional needs and diet and fluid intake for resident as prescribed
  • Keep medication files and maintain inventory of all prescribed medications; order medication and supplies as needed
  • Assist with the assessment, development, and evaluation of a resident individualized service plan based on resident needs and medical condition; use care plans as a basis for providing therapeutic care to resident
  • Monitor resident response to treatment plans and identify adverse medication interactions and notify Clinical Director and other clinical and medical staff as deemed appropriate
  • Create, update and maintain accurate and up-to-date resident case files and records with information pertaining to nursing care, child’s medical history, treatment plans, and observations; chart all pertinent information in the resident case files/records in accordance to Agency policy and licensing standards
  • Work closely with clinical and program team to admit, transfer and discharge resident as directed
  • Support and enforce infection-control policies and procedures
  • Use assessment information and resident care during the shift to develop a care plan that accurately communicates the required and needed information for incoming shift to properly and adequately care for the resident
  • Respond to all crisis situations in a timely manner; ensure all internal and external parties are properly notified in a timely manner; report incidents according to licensing standards
  • Seek guidance when encountering nursing procedures and responsibilities that have not been mastered
  • Assist with the safe and physical evacuation of resident during an emergency situation or natural disaster
  • Provide on-call services when needed
  • Complete insurance forms and other paperwork as it relates to treatment plans, placement and discharge summaries and administration of medication
  • Complete monthly, quarterly and annual reports as requested and to remain in compliance with licensing and funding standards
  • Plan, schedule, and facilitate trainings for staff, such as blood borne pathogens, CPR and First Aid
  • Provide on-call services when needed
  • Other duties and special projects as assigned

Working Relationships

  • Develop and maintain strong working relationships with management, staff, residents, families and sponsors, community partners and professional service providers
  • Work closely with the Clinical Director and staff to ensure residents are receiving the highest quality of care and the program is operating efficiently and effectively and is meeting the goals and objectives of the program and the needs of the resident
  • Work as team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practices

Minimum Qualifications

  • Current LVN/RN license to practice in the state of Texas
  • 1-year nursing experience
  • 1 year supervisory or management experience
  • Ability to maintain a professional approach and build a rapport and trust with residents and their families
  • Consistently follows established standards, policies, and procedures in providing nursing care
  • Strong writing skills with the ability to document and maintain accurate resident files/records
  • Strong customer service, interpersonal, and people skills; ability to work and interact with diverse groups of people and handle stressful and emotional situations
  • Strong relationship management skills with the ability to work with a team in a collaborative manner
  • Strong time management and organizational skills with the ability to pay very close attention to details and meet deadlines
  • Ability to resolve conflict and respond to resident issues with professionalism, tact, and empathy
  • Proficient in use of personal computers, Microsoft Word, Excel, and Outlook, and information systems used to document and record resident information

Preferred Qualifications

  • 2 years’ nursing experience
  • Nursing experience with pediatrics and adolescents, highly preferred

Physical Demands & Work Conditions

  • This position will require continuous viewing of a computer screen, sitting and standing for long periods of time and frequent walking.
  • Position works in a child facility and has constant exposure to children
  • Lift, push, pull, move up to 50 lbs.
