Description:
Under general supervision, performs psychosocial assessments, treatment planning/monitoring and discharge planning. Provides counseling/therapy to clients requiring complex intervention. Performs commitment determinations and/or executes orders of involuntary commitments.
Deliver services including assessment, treatment planning and therapy to individuals with Severe and Persistent Mental Illness (SPMI) in the community as part of a multidisciplinary team. Complete concurrent documentation and submit authorizations in a timely manner. Coordinate with other providers to ensure quality intensive care is delivered. Services are performed in unstructured settings including homes, prison/jail, hospital/CSU.
PREFERRED QUALIFICATIONS: Experience working in community-based care of individuals with SPMI, particularly experience as part of an ACT Team. Preference will be given to applicants who demonstrate excellent computer skills and have documented experience working in electronic health records.
Must hold current license for a LAMFT, LMFT, LAPC, LPC, LMSW, or LCSW or be eligible to be licensed.
NOTE: McIntosh Trail Community Service Board conducts background checks on all final candidates. Hiring of applicants is contingent upon satisfactory results of:
- Fingerprint Based Background Check of the Georgia and Federal Criminal History Record Database
- Drug Screen
- Motor Vehicle Report (MVR) for seven years
TB testing will be required upon appointment. If you have previously tested positive for TB, you will be required to provide a copy of your chest x-ray prior to your first day of orientation.

Qualifications:
Master's degree in a related area from an accredited college or university AND eligibility to be licensed AND two years of supervised experience in the area of assignment.
