The Program Specialist – Population Health Integration supports and facilitates program development, implementation, ongoing performance evaluation, continual process improvement and operational integration for Population Health. This position will specialize in facilitation and management of business program and process design and improvement activities, working collaboratively with business owners, clinical analytics, process improvement, project managers and subject matter experts. The position will act as project manager on select program initiatives tied to the Population Health roadmap responsible for developing project plans, managing project efforts and maintaining appropriate documentation needed for key decisions and implementation. This position will partner with Population Health and clinical leaders to link operational performance to organizational goals facilitating ongoing evaluation and improvement activities. As institutional knowledge increases, will act independently as a SME on various efforts outside of Population Health.
Manages and supports projects across SSM Health Wisconsin in support of value-based care initiatives and the Population Health Road Map.
Assists in the development and maintenance of the Population Health strategic roadmap, as directed
Implements new Population Health strategic initiatives; establishing scope, timelines, milestones, monitoring work across departments and entities, identifying operational limitations/constraints, identifying impacted systems, and business workflow
Creates and maintains project documentation for Population Health program initiatives including project charters, project plans, value/impact assessment and other program documents as needed
Facilitates cross-functional meetings to troubleshoot issues and develop resolution plans.
Organizes and facilitates work sessions with stakeholders within and across SSM Health Wisconsin, to gain understanding of current business processes evaluating opportunities as it aligns to population health goals and strategies
Re-engineers processes to ensure business needs are met and efficient processes are implemented.
Assists staff in initial creation and ongoing maintenance of process, procedure and training plan documentation for Population Health programs.
Trains clinical teams as well as other key areas within SSM Health Wisconsin on new programs and/or process changes.
Works with Clinical Analytics and Medical Economics to prepare reporting for senior leadership on new initiatives and existing Population Health program performance including agreed-upon metrics and dashboards
Acts as coordinator for activities with shared responsibilities such as contract reviews and request for proposals
Works closely with clinic and business areas across SSM Health Wisconsin including Clinics, Finance/Accounting, Marketing/Communication and Center for Clinical Analytics to identify, catalog and monitor requirements for reports, meetings, decisions and analysis.
Acts as a SME and representative for Population Health with a variety of implementation teams on efforts related to Q&CM.

Bachelor’s degree with a minimum of two years of combined experience in health care related work, health insurance technology or process improvement OR an Associate Degree with 7 years of related experience in health care related work, health insurance, technology or process improvement OR a high school degree or equivalent with 10 years of relevant work experience.
Previous experience in leading projects, process improvements, training or representing a department while working as part of a cross functional or implementation team
Ability to work effectively with Managers, Directors and above, as well as through the organization
Ability to research, collect data/information and implement a process
Excellent organizational skills, and solid business judgement
Self-motivated, self-starter and ability to work independently as well as working effectively in a team environment
Excellent verbal and written communication skills, with strong attention to detail
Strong analytical thinking and problem solving skills
Valid driver’s license and personal, reliable vehicle required for work related travel to fulfill responsibilities
Ability to quickly gain familiarity with business concepts, systems and workflow
Proficient computer skills including Microsoft Office (Word, Excel, Outlook, PowerPoint)
Six Sigma Certified Green or Black Belt
Experience in population health programming

Based in Madison, Wis., Dean Clinic consists of a network of more than 60 clinics in south-central Wisconsin. Our more than 500 physicians provide primary, specialty and tertiary care in the clinics as well as eye care through our Davis Duehr Dean locations. Dean Clinic also offers urgent care services and operates outpatient surgery centers. Dean Clinic joined SSM Health in 2013.
