Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.
Entry level position, with primary function and responsibility being patient care.
1. Patient Assessment Criteria A:
Assess patients according to department guidelines (refer to Policy and Procedure Manual, Clinical Pathways and Age Appropriate Criteria). Patient Assessment Criteria B: Establishes appropriate, measurable and functional goals based on evaluation.
2. Documentation Criteria A:
Documents all patient care according to department policy. Documentation Criteria B: Writes notes in a legible and organized manner. Documentation Criteria C: Writes notes in a timely manner in accordance with department policy.
3. Patient Treatment Criteria A:
Treats patients according to department guidelines (refer to Policy and Procedure Manual, Clinical Pathways and Age Appropriate Criteria). Patient Treatment Criteria B: Meets productivity standards according to program policy.
4. Discharge Preparation Criteria A:
Anticipates/initiates patient discharge needs related to equipment, environmental barriers and community re-entry. Discharge Preparation Criteria B: Provides patient education and appropriate exercise handouts. Discharge Preparation Criteria C: Provides training to family/caregiver according to policy. Discharge Preparation Criteria D: Attends and contributes to team conferences on patient progress and discharge needs as per program requirements.
5. ACRM Core Values Criteria A:
Demonstrates the seven core values of ACRM: flexible, dependable, efficient, team player, compassion, integrity, respect. ACRM Core Values Criteria B: Demonstrates flexibility in covering other services and performing other duties as directed.
6. Performs other duties as assigned.
7. Fulfills all compliance responsibilities related to the position.
Connecticut licensure to practice discipline or eligible for licensure. 1 year of experience preferred with primary function and responsibility being patient care. Proficiency in specialty areas such as Ortho, Neuro, Lymphedema or Hand Therapy preferred.
Weekend coverage required.
