Merakey is looking for a Full-Time Transitional Coordinator to join our Carlisle, PA Team

POSITION REQUIREMENTS

  • Must possess a Masters degree in a human services field with one year of experience working with children and young adults
  • Or Bachelors Degree in a Human Services field, with minimum of 2 years experience working with children and young adults.
  • Candidate must possess understanding of Child and Adolescent Service System Program (CASSP) principles and resiliency concepts.
  • Must possess a valid, PA Driver's License and access to reliable transportation.
POSITION OVERVIEW

  • Engage youth and young adults in their own future planning process.
  • Provide developmentally appropriate services and supports.
  • Involve youth, their families, and significant others in a process preparing and facilitating the youth's movement toward adulthood.
  • Provide individual sessions for transitioning youth on decision-making skill building, education and career choices, and community life functioning.
  • Engage youth in small group activities based on individual support plan goals.
  • The objective is to help young adults successfully transition into adulthood by making sure they have skills for employment, living independently and are involved in the community.
