Nurse Extern (25002)

Must be entering their final year of Associate or Baccalaureate RN program of an accredited School of Nursing.
Obtain certification in Basic Life Support within 30 days of hire date.
The nurse extern is a temporary position for nursing students to practice in all nursing areas. The nurse extern is co-assigned with a RN and may perform direct and indirect patient care under RN supervision.
Essential Duties and Responsibilities:
Include the following. Other duties may be assigned.
1.0 Assists the registered nurse in the practice of professional nursing by providing direct patient care to promote patient comfort and facilitate the performance of diagnostic and treatment procedures as outlined in the nursing care plan.
1.1 Assists the RN with the patients activities of daily living according to the needs of the patient in a timely manner, following departmental procedure.
1.2 Performs designated procedures as needed by following departmental procedure.
1.3 Accurately observes, records, and reports to the RN all patient concerns, symptoms, reactions, and progress in a timely manner.
1.4 Assists the RN with examinations and procedures according to departmental procedure.
2.0 Demonstrates competency in basic and intermediate care skills as verified and validated by nurse mentor, clinical preceptor, nurse manager and/or nurse director. Assists the RN with procedures defined by the advanced skills inventory.
2.1 Demonstrates competency in those skills identified on the skills inventory that may be carried out independently.
2.2 Demonstrates competency in those skills identified on the skills inventory that may be carried out under direct supervision of a RN.
3.0 Educates self effectively and assists in peer education. Seeks educational opportunities to increase competence. Attends staff meetings and unit in-services as applicable.
4.0 Assists the RN with indirect care by maintaining a clean, sanitary, and safe environment to prevent and/or control infections and to provide for safety of patients, visitors, and employees.
4.1 Thoroughly cleans assigned patient care areas according to departmental procedure.
4.2 Thoroughly cleans assigned unit equipment according to schedule and/or need, and keeps hallway and work areas free of equipment according to departmental procedure.
5.0 May act in the role of a sitter for a patient as assigned by the charge RN.
6.0 Other related duties/ projects as assigned.
Unlicensed assistive personal (UAP) may have limited access to mediation including during transport of non-controlled substances at the request of a licensed care provider.

Nurse Extern Skills List
Nurse Externs are not permitted to perform:
- IV insertions, blood draws or medication administration
Basic skills that may be performed independently by the Nurse Extern once competency has been demonstrated and documented are as follow:
- Ambulate patient
- Position patient
- Back rub
- Dress patient
- Feed patient
- Make unoccupied bed
- Obtain vital signs
- Obtain height and weight
- Oral hygiene
- Bath patient
- Proper use of beds (i.e. weights)
- Transport patients/items
- Perform chest PT
- Maintain safe environment
- Document VS and I/O’s
- Take and listen to report
Intermediate skills that may be performed by the Nurse Extern under direct supervision of co- assigned RN once competency has been demonstrated and documented as follows:
- Apply binders
- Check NG/OG placement
- Post mortem care
- Care for patients with oxygen
- Collect random/clean catch urine specimens
- Collect stool specimens and heme test
- Monitor IV infusions
- Perform urine dipsticks
- Perform rectal stimulation
- Administer enemas
- Care for patients in isolation
- Clamp, connect/disconnect urinary catheters
- Perform finger sticks for blood glucose
- Complete pain evaluations
- Place and manage soft restraints
- Recognize S/S of peripheral line infiltration
- Evaluate skin break integrity
- Collect patient data
Advanced skills that the nurse Extern may assist the co-assigned RN with but may not perform independently are as follows:

- Develop nursing diagnosis
- Patient teaching
- Contribute to IPOC
- Admit/Discharge patients
- Oral and tracheal suctioning
- Trach Care for patient with radioactive isotopes
- Dressing changes-clean/sterile
- Foley catherization
- Colostomy/ileostomy care
- Continuous and intermittent bladder irrigations
- Administer feeds via peg tubes and NG/OG
- Insert NG/OG and check placement
- Care for patients with chest tubes
- Obtain supplies for bedside procedures
- Open sterile supplies
- Care for patients with traction
- Access hospital-paging system
- Give report
Nurse Externs are expected to document all care / interventions provided independently within the scope of an unlicensed assistive personnel.
