CSS Program Manager
Intake community support staff will provide initial case management assessment
and session at time of outpatient intake that consists of completing a Community
Needs Assessment (housing, employment, food, etc.), assistance with Medicaid and
disability applications/paperwork, and general client engagement. The CSS
program provides services that consist of rehabilitative skills building, the
development of environmental supports and resources coordination considered
essential to assist a person in improving functioning, gaining access to
necessary services and in creating environments that promote recovery and
support the emotional and functional improvement of the individual.
DUTIES AND RESPONSIBILITIES
Staff will serve as the assessor and initial program linkage coordinator for new
clients completing their initial intake for behavioral health services.
Assessment will identify need for linkage to community, general entitlements,
and psychiatric, substance use/abuse, medical services, crisis prevention and
intervention services. Staff will also assist clients to complete Medicaid and
SKILL, COMPETENCY AND EDUCATION AND/OR LICENSURE REQUIREMENTS
A minimum of a Bachelor degree, or may hold certifications in related areas of
expertise to include SOAR, Addiction Counseling and Psychiatric Rehabilitation.
Must have a current driver’s license. SOAR certification is preferred.
Able to work closely with other staff within center, including clinicians,
psychiatrists, and front desk staff, as well as other community providers for
client referrals and linkage. Ability to interact and communicate with clients
and their families effectively.
Primarily office-based at the Winn Way Mental Health Center Monday – Friday
Must be able to work with diverse populations and culture of people, able to
communicate effectively, multi-task, be self-motivated and organized.
