- The Support Specialist (SS) reports directly to the Service Implementation Manager (SIM) and is responsible for supporting the coordination of services for up to 40-50 individuals. The Support Specialist is a hands-on role ensuring that each person within a “community group” is receiving services as outlined in their Person-Centered Plan (PCP). Additionally, the SS will provide support to Community Support Professionals (CSP) around the creation of community-based activities for people is services. The SS will work closely with the PCP Specialist and SIM to ensure that meaningful day activities include items that are focused on skill building and goal achievement.
What you'll do
- Train and develop Community Support Managers (CSM) and CSPs on PCP plans, escalation, and baseline needs.
- Provide field-based oversight of CSP activities monthly.
- Attend progress reviews for each person on caseload.
- Follow incident reporting procedures when necessary.
- Communicate and collaborate with Community Support Professionals and Managers for meaningful activities.
- Inform Opportunities, Inc. staff of individual needs related to services.
- Coordinate with internal and external partners for smooth service delivery.
- Identify and connect Natural Supports in a variety of community-based settings where people in services frequently participate.
- Gain knowledge of public transportation systems to support individuals on caseload with learning how to grow independence with transportation.
- Conduct regular community assessments to identify and leverage local resources that can enhance and support our services.

Qualifications

- Bachelors degree in counseling, Social Work, or related field
- Abreast with Montgomery County and/or Prince George’s County activities and resources
- A valid driver's license, clean driving record, reliable transportation and be willing to drive company vehicles.
