POSITION PURPOSE:
The Care Coordinator is a Certified Medical Assistant which is a non-exempt position responsible for clinical and administrative duties to support nephrologists and advanced practitioners in the delivery of care to patients with Chronic Kidney Disease (CKD). Duties may include but are not limited to gathering and organizing patient data. The Care Coordinator works to identify patients’ unmet needs, engage patients in their own care, gather summary information for treatment interventions, and enhance ongoing communication between the patient and her/his care team. The goal of the Chronic Care Management program is to facilitate high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment. Each Care Coordinator will be expected to manage a 500-patient caseload with 300 billable by the end of month.
B. PHYSICAL DEMANDS:
The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of the job. Physical requirements can vary and must be reviewed by management. In general, this position requires the ability to lift 5-50 lbs unassisted and the ability to stand, sit, stoop, walk, stretch, and use full range of body motions. Manual dexterity as required for specific equipment and ability to use computer keyboard and peripherals.

C. WORK ENVIRONMENT:
Business office with computers or printers, light traffic. Required tasks involve exposure to bodily fluids and tissue than would be encountered by a visitor.
D. Education:
High School Diploma
Vocational/Technical School Certificate Degree
Experience:
0-2 years' experience as a Medical Assistant
Notes: certification sponsorship available for qualified applicants and to be obtained within the first 60 days (about 2 months) of employment.
F. SPECIALIZED REQUIREMENTS:
A background check (which may include: checks of employment records, education records, criminal records, civil
records, motor vehicle records, professional licenses, and sex offender registries, as position requires) must be completed satisfactorily before any candidate can be offered a position with Innocura. policies and procedures.
G. PREFERRED QUALIFICATIONS:
Ability to travel between clinic locations is required. Open availability is preferred, but not required.
Comfort in EMRs and practice systems such as pMD, CKD EHR by Epic, and Cerner is preferred, but not required.
KEY AREAS OF RESPONSIBILITY:
● Develop a keen understanding of our nephrology practice, requirements for optimal, coordinated population health
● Work as an effective team member of the care team
● Collaborate with care teams to establish population-appropriate, pre-visit, and point of care processes
● Work with the Clinical team and Chronic Care Management platform to support patients with chronic disease and assists in coordination of the patient’s care continuum
● Contribute to quality improvement and care redesign of population health efforts
● Manage patient registries
● Provide the members of health care teams in designated practices with the data required to meet the health needs of the patient
● Support practice staff to develop interventions to proactively manage target populations
● Contribute to a positive experience for patients and families through courteous telephone and digital interactions, accurate and expeditious routing, as well as referrals to appropriate clinical staff when necessary
● Recognize and report data inconsistencies to appropriate personnel
