Halifax Health is seeking a Care Manager for our Deltona Location.
This position performs case management which includes utilization review, social services and discharge planning that will provide the appropriate utilization of services and appropriate post hospital care for assigned group of newborn, pediatric, adolescent, adult and geriatric patients.
Formal Education/Current Florida licensure: RN, LPN, Social Work, or other social services related field (Masters preferred).
Professionalism in interpersonal communication skills with patients, patient families, colleagues, physicians and ancillary department personnel required.
Personal professional development, accountability, organization and leadership in the performance of professional nursing practice.
Must have good interpersonal communication skills.
The ability to organize, prioritize, analyze, design and implement plans is a necessity.
The ability to handle multiple responsibilities and tasks in stressful situations.
