Right now, we have an exciting opportunity for an Outpatient Nurse Coordinator to provide exceptional, compassionate care at our Children's Heart Center. We're looking to you to provide direct patient care as well as coordinate CCS Cardiac Special Center Care team services, Fetal Cardiology services, and Single Ventricle Program services. You will also lead quality improvement efforts as part of The National Pediatric Cardiology Collaborative. You will coordinate care for patients with prenatally diagnosed congenital heart disease by communicating with multiple specialties and developing the plan of care for delivery and the immediate postnatal period. This involves close collaboration with physicians, clinical staff, and administrative staff.
We're seeking a highly-skilled, self-motivated professional with:
BSN or MSN and current California RN licensure
Three or more years of pediatric and cardiology nursing experience
Familiarity with the CCS and MediCal programs
Ability to coordinate patient care, using a team approach
Expertise in patient/family education
Strong communication skills - bilingual (English/Spanish) preferred
Ability to establish and maintain cooperative working relationships
Technical writing skills to document assessment of patient response to therapy
Background in data collection and analysis
Computer proficiency with Word, Excel, and PowerPoint
Exceptional interpersonal and team work skills
