Job Summary:
Provide client care and services management to assigned residential clients within specified region to insure optimal client care.
Job Duties & Responsibilities:

Insure all properties remain in compliance with all applicable standards and regulations.

Insure all individuals receive care that is in compliance with all applicable standards and regulations.

Work with Property Managers, Support Coordinators, Medical Providers, Social Security, DFCS, Adult Protective Services, Mental Health, and other Service Providers to provide optimal client care.
-
Work closely with Support Coordinators, medical physician and staff, mental health, and other various support systems that ensure individuals receive all waiver services, as outlined in ISP.

Insure all client rights are appropriately respected and honored as well as report any violations.
-
Conduct visits to all properties for scheduled and unannounced review of client care at least weekly and as needed when clients are on premises.

Insure Health Risk Screening Tools (HRSTs) are updated annually and as needed for changes in diagnosis, medications, physician visits and hospitalizations.

Create and develop training material for individualized HRST training recommendations.
- Attend ISP reviews and provide adequate feedback regarding individualized services and care.
- Manually enter ISP reviews in Therap before assigned deadline.
- Attend ISP Addendum meeting when needed.

Review client goal tracking to ensure that all services are provided and documented in accordance with client ISP.

Attend SIS meetings and provide adequate level of supports needed to ensure appropriate funding per individual.
- Attend all Behavior Support Plan meetings and provide staff training when needed.
- Monitor and review electronic documentation daily.

Insure electronic client records, medications, and monthly MARS are accurate and accessible to staff.
-
Maintain contact with client’s families as needed and required.
-
Maintain current client records on-site at each property.
-
Ensure client’s Medicaid is current and up-to-date.
- Ensure individual food assistance program cases are current and up-to-date.
-
Insure all annual paperwork is complete and filed in clients binder(s).
- Review all assessments and insure follow-up on all recommendations provided by Regional I&E Teams.

Attend client medical appointments as needed.
-
Monitor and insure client health, safety, development and quality of care.
-
Complete General Event Reports and/or Critical Incident Reports, as necessary, and submit within established guidelines.
-
Assist individuals with community involvement and natural support building through activities, outings and other events.
-
Utilize community and any other resources available to insure all individual needs are met.
-
Ensure Billing is approved by site, by assigned deadline.
- Utilize SilverLining to monitor onsite vehicle usage.
-
Work to identify, create and submit Exceptional Rate Requests and Renewals as needed for additional funding and to insure appropriate funding.
-
Review client satisfaction survey results & monitor implemented changes.
-
Train and supervise site leaders and direct care staff (DSP and LPN) on individualized client care and aid.

Monitor staffing for the region and each site to address staffing needs and collaborate with HR to screen, interview and select qualified staff.
- Insure that staff is in place to provide all approved services for the region as scheduled.

Insure that all staff receives proper introductory training and completes continuing education requirements.
-
Insure that all staff maintains current required certifications including CPR, First Aid, Defensive Driving, Professional Certification and Licensing, TB clearance and Crisis Prevention Intervention.

Schedule, plan and conduct monthly team meetings.
-
Manage work schedules, duties and responsibilities for all staff.
- Provide on-going, effective performance management to all staff including progressive discipline and annual evaluations.

Assess training needs and provide information to responsible parties to insure compliance.

Operate properties within projected budget to insure program efficiency.
-
Approve and submit travel reimbursement for region for all travel.
-
Maintain cleanliness and functional operation of local satellite office.
-
Complete continuing education as required for growth and development.
-
Insure submission of required documents to various departments as scheduled.
-
Collaborate with Program Director for intake and evaluation of potential clients.
-
Interact with Regional Boards, Clients, Families and ESSG staff including attending board meetings and other leadership responsibilities as appropriate.

Follow Health Insurance Portability and Accountability Act (HIPAA) privacy rule to insure client protected health information is secure.
-
Maintain knowledge and understanding of Easter Seals Southern Georgia policies and procedures.
-
In the absence of the Director and Assistant Director of Residential Services, may assume duties and responsibilities of this position.

All other duties as assigned.
Essential Functions:
-
24 hours a day / 7 days weekly availability for emergency situations only.
-
Travel to multiple ESSG properties for site management, meetings and events.
-
Ability to lift, push and pull a minimum of 50 pounds.
-
Ability to speak publicly for various groups to advocate for ESSG mission, vision, values and clients.
-
Use of technology and office equipment including: laptop, email, Smartphone, Microsoft Office Suite, scanner, fax, printer, PolicyStat system, THERAP system, Client Information System (CIS), Health Risk Screening Tool (HRST) and other client records management systems.

Minimum Qualifications:
-
Bachelor’s Degree in Sociology, Psychology or related field of study.
-
2 years professional work experience in residential disability services.
-
2 years managerial experience.
- CPR/AED certification.
