Alcona Health Center has a current opening for a LPN Care Manager for our Substance Use Disorder Program. This position has the flexibility to be stationed in Cheboygan, Harbor Springs, or Indian River as the home office. However, travel will be required to all of our northern locations.

The Care Manager is a practice-based LPN who directly supports AHC’s highest risk patients. In collaboration with other members of the healthcare team, the LPN Care Manager for the SUD Program is responsible for organizing, coordinating, and providing care coordination and care management services to patients within the practice who are most at risk for health deterioration, sentinel events, and/or poor outcomes.
PRIMARY RESPONSIBILITIES AND DUTIES
Patient Care Activities
- Provide Substance Use Disorder nurse case management for multiple integrated primary care teams on a dedicated schedule.
- Conduct comprehensive assessment of patients’ physical, mental, and psychosocial needs
- Develop care plans to prevent disease exacerbation, improve outcomes, increase patient engagement in self-care, decrease risk status, and minimize hospital and Emergency Room utilization.
- Utilize behavioral strategies help patients adopt healthy behaviors and improve self-care in chronic disease management. Promote self-management goals.
- Assist patients in navigating the health care system. Coordinate Specialty care, follow-up on test results and other care coordination needs.
- Partner with external case management programs to coordinate care.
- Ongoing evaluation and documentation of patient progress/ risk status.
- Participate in risk rounds to assess high risk patients.
- Complete follow-up appointments for providers due to missed appointments or provider absence.
- Provide overdose and Narcan training to patients and family members.
Patient-Center Medical Home
- Pro-actively support Patient Centered Medical Home initiatives related to care coordination
- Pro-active member of care teams in team-based care initiatives
- Partner with Patient Centered Medical Home staff to develop integrated care management programs
Communication and Health Information Entered into Patient Record
- Enters appropriate treatment information into patient record in a timely fashion.
- Progress notes documentation is completed accurately, is written and maintained in a manner that is clear, complete, current, and organized in accordance with state and federal regulatory requirements.
- Progress notes entered into electronic record to insure appropriate ongoing patient care.
- Continuously review charts for new and/or relevant information concerning clients.
- Communicates patient’s health status to Care Manager RN to insure adequate provision of care.
- Routinely speaks at site, Medical Support Staff, and provider meetings to remind staff of the services offered through Care Management and to encourage referrals from all staff.
- Works collaboratively with medical and behavioral health providers to enhance holistic care of patients within the Substance Use Program.
- Participate in regularly scheduled (usually biweekly) caseload consultation with the Substance Use Program care team members and communicate resulting treatment recommendations to the patient’s medical provider.
- Maintain registry for Substance Use Program for assigned region.
- Completion of grant-related paperwork and data entry into database.
- Ensures attendance and hours worked are accurately recorded.
- Properly manages paid vacation and sick leave.
- Responsible for regular, predictable attendance and to work hours as scheduled.
Maintains confidentiality of protected health information and safeguards all patient related information at all times.
Consistently adheres to organization rules, regulations, and policies.
Attends/participates in departmental meetings.
Accepts and performs other job related duties, projects and responsibilities, as required.

Licenses and Certifications
- Possesses a current Michigan state licensure to practice nursing (LPN).
- Possesses a current Basic Cardiac Life Support (BCLS) CPR certification.
- Possesses a current driver’s license and automobile insurance.
Technical Skills & Abilities
- Ability to prepare correspondence and reports in Microsoft Word.
- Ability to create, and send and manage email in Outlook.
- Fully functional in the use of the Electronic Health Records system.
