Cobble Hill Health Center is a not-for-profit long- and short-term care facility providing services to the disabled and elderly for over 40 years. The Director of Social Work will play an integral role in helping us to achieve our vision: to consistently exceed expectations and set the standard for excellence in residential and community care.
As the Director of Social Work, you will lead the services and activities related to the social services department. You will be a key leader in developing strategies and planning to address resident needs and supporting the overall objectives of the organization.
Who you are:
A leader who will lead, supervise, and assign activities to social work staff including:
Developing and writing social work policies and procedures
Evaluating service activities of social work staff, including chart audits and advanced directives
Managing resident case load, completing initial psycho-social assessment and progress notes
Participating in care plan meetings
Supervising activities of student MSW interns
Educating and assisting staff on resident's rights, advanced directives and resident sexuality
Overseeing discharge planning and maintain statistical information
Coordinating social work activities with other departments
Participating in resident council, department head, MDS, UHF, quality assurance and various other committee meetings
Assisting with and overseeing others in the resolution and investigation of resident occurrences
Overseeing the education of residents and families regarding palliative and hospice care
Completing and supervising others in completing screens and PASRRS for MI and MR residents
Participating in determination hearings
Working on various community presentations
Coordinating social work, finance department, and residents/family regarding Medicare Part D
Completing payroll process weekly
Managing and providing required Department of Health and other agency required reports and audit information
Performing Administrator on Duty service on a rotating basis
Performing other related duties as assigned
What you offer:
Licensed Master's in Social Work required
Extensive and diversified background with at least 10 years of related experience
Experience in Skilled Nursing Facilities required
Understanding and practical application of practices, theories, and policies related to social work
Superior verbal and written communication and interpersonal skills
Superior managerial and diplomacy skills
Proficient in Microsoft Office Suite
Excellent organizational skills and attention to detail
Excellent analytical, decision-making, and problem-solving skills
Ability to assess and manage priorities
Ability to manage varying volumes of work load and pivot as appropriate
Prolonged periods sitting at a desk and working on a computer and movement around facility
Must be able to lift up to 15 pounds at times.
