Banner University Health Plans Dept of Innovation and Collaborative Care. We are responsible for implementing Triple Aim programs to improve the quality, cost, and experience of care. We are in a major growth phase with State AHCCCS and MACRA regulations driving the rapid conversion from Fee for Service to Value-based payment. These relationships require contractual development and monitoring, onsite provider support and improvement planning, and collaboration across statewide organizations and Banner-wide departments.

As a Health & Wellness Coordinator you will be responsible for the coordination and management of some of the many provider partnerships we are contracting with across central and southern Arizona. In this position you will manage the external (Healthy Together Hub )in partnership with BUMG's Healthy Together home-based interdisciplinary team-a commitment we made in our 2017 ALTCS and 2018 Complete Care AHCCCS bids.

Job Summary

This position is responsible for developing, implementing, performing, coordinating, and evaluating a wide range of health promotion activities and programs for targeted populations.

Essential Functions

Develops, coordinates and performs facility based health promotion services, activities, and programs including, screenings, health risk assessment, educational presentations and classes, intervention programs, outreach activities, and alliances to enhance community and business networks.

Schedules and prioritizes service/client requests to contribute to efficient and cost effective operations. Demonstrates proper materials management skills and purchasing procedures.

Represents hospital and hospital-based services to internal & external publics, creating appropriate referrals and cross-selling opportunities for all service lines.

Works collaboratively with other health promotion coordinators and wellness staff in the development, implementation and evaluation of health and wellness program designs and scheduling of program events.

Provides leadership, consultation, budgeting input, education and planning services to the organization.

Executes administrative duties including independent decision making in the delivery of services and assisting in the management of Massage Therapists, Wellness Specialists and/or other local instructors and health professionals.

May be assigned to supervise staff.

May be responsible for overseeing the planning, implementation and evaluation of the DSME program. Serves as a liason for the program between the staff, the Advisory Group, and other departments. Maintains ADA recognition and participates in the evaluation of the program's effectiveness.

Minimum Qualifications

Requires knowledge typically obtained with completion of a bachelor's degree in Exercise Science, Kinesiology, Corporate Recreation, Health Promotion/Wellness or a related field.

Must have three or more years of experience in the health promotion/occupational health area in addition to experience in conducting fitness and occupational health screens, program planning, development, coordination and evaluation. Demonstrated leadership success in activities such as program development or management activities is required.

Must be able to work effectively with all common office software.

Preferred Qualifications

Prior supervisory and leadership experience is preferred.

Additional related education and/or experience preferred.
