A combination of sound medical case management knowledge and ability to go with the flow and adapt to each unique situation is vital.

Job Description – Field Nurse Case Manager

This position is responsible for providing medical case management services that promote efficient coordination of care. The Field Nurse Case Manager will assist all stakeholders in developing successful return to work interventions for injured employees across diverse employment venues. In conjunction with developing and coordinating injury recovery strategies and return to work plans, the Field Nurse Case Manager is responsible for submitting timely case reports and updates to insurance claims adjusters.The Nurse Case Manager performs his/her job functions in a home-based office and in the field. The position requires frequent travel to meet with medical providers, attorneys, employers, and injured workers.

Essential Duties and Responsibilities

  • Collaboratively assess injured workers’ needs with medical providers to coordinate treatment plans that facilitate the injured employee’s return to health enabling the worker to return to gainful employment
  • Develop and implement cost savings strategies that maintain high-quality medical care while ensuring favorable treatment outcomes
  • Establish and maintain productive relationships with medical providers, claims adjusters, attorneys, employers, while advocating for injured workers to ensure favorable recovery outcomes
  • Provide detailed and timely case status information to claims adjusters as needed
  • Devise and implement successful return to work strategies for the employee with the employer utilizing Windham Group vocational and ergonomic services
  • Provide education and counseling to injured employees and their families about the workers' compensation system as well as medical team members to help achieve the goals of case management services
  • Document in detail all communications, actions and outcomes utilizing Windham Group’s proprietary case management software program
  • Document and capture all professional case management tasks performed in Windham Group’s proprietary billing software program
  • Ensure the privacy of all medical and privileged information related to an injured employee

Requirements

  • Be a graduate of an accredited school of nursing (bachelor’s degree preferred)
  • Active registered nursing license in NJ, additional PA state licensing preferred
  • CCM Certification a plus
  • Ability to travel throughout geographic region of responsibility (within a 2 hour radius of your home location)
  • Dependable personal transportation
  • Strong clinical background in orthopedics, med-surg, emergency room, and/or occ health
  • Computer literate, able to type, able to organize and construct electronic files
  • Working knowledge of Microsoft Word, Microsoft Outlook, Adobe and smart phones
  • Excellent communication skills; oral, written and reading
  • Bilingual (Spanish) highly desired
  • Ability to prioritize, working on several cases simultaneously
  • Excellent time management skills
  • Knowledge of managed care mandates and workers’ compensation case management
  • Ability to develop trust and positive relationships with coworkers, customers, injured workers, employers and medical professionals
  • Ability to work both independently and within a team environment
