GENERAL SUMMARY:
The LPN is responsible for assessing clients, documenting observation and assessment data, evaluating outcomes, and implementing the nursing plan of care. Implements the medical plan of treatment and follows physician’s orders. May provide hands-on, direct clinical care. Receives clinical direction and oversight from a Registered Nurse. Performs duties within the scope of the Nurse Practice Act regulations, the State Board of Nursing, and policies and procedures of the company.
ESSENTIAL JOB RESPONSIBILITIES:
Completes orientation and competency assessment/verification as required.
Achieves client-related skills competency as verified by supervisor
Attends required inservices as necessary to maintain employment.
Maintains expiring requirements including professional license and required certifications.
Consults, seeks advice, and follows direction of the RN in fulfillment of care of the client.
Ensures the provision of nursing services as outlined in the service plan of care.
Consults with client’s physician and obtains medical orders when needed to ensure the delivery of safe, adequate, and appropriate skilled care. Documents orders as required.
Anticipates potential problems, identifies emerging problems, and acts accordingly.
Communicates effectively and routinely with supervisor, ensuring that pertinent information reaches supervisor in timely manner. Report changes in client’s health to supervisor in timely manner.
Works closely with other health professionals/providers in planning, evaluating, and implementing services to address the health needs of individuals served.
Establishes and maintains professional rapport with clients, families, and other health providers.
Cooperates with other individuals and agencies involved in client care to ensure continuity of care is maintained.
Seeks guidance and assistance from supervisor when necessary.
Collects and records data in systematic manner, using appropriate forms.
Completes all required documentation in a timely manner consistent with company standards.
Cooperates in external / internal investigations as requested.

QUALIFICATIONS/EDUCATION
Graduate of an accredited school/college of School of Practical Nursing with current registration in state of primary work site location as a Licensed Practical Nurse.
Current certification in Basic Life Support (BLS/CPR) required. Certification in Advance Cardiac Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) may be required
Two years experience as an LPN required at a minimum.
LPN must never have been shown by credible evidence (for example, a court or jury, a department investigation, or other reliable evidence) to have abused, sexually assaulted, exploited, or deprived any person or to have subjected any person to serious injury as a result of intentional misconduct as evidenced by an oral or written statement to this effect at the time of application.
