About the Role:
Are you a skilled Registered Nurse (RN) who enjoys making a difference in patients’ lives through home-based care? We are seeking experienced nurses to perform health assessments, care coordination, and supervise non-skilled care providers in a compassionate and flexible environment.
Key Responsibilities:
- Conduct Initial Health Assessments and develop comprehensive Care Plans tailored to each patient.
- Schedule and perform Supervisory Visits for Certified Nurse Assistants (CNAs), caregivers, sitters, and Personal Care Aides (PCAs).
- Provide ongoing Care Coordination to ensure high-quality, patient-centered care.
- Drive to patient locations as required to deliver services in person.
- Ensure all nursing services adhere to state and local regulations, the Nurse Practice Act, and accreditation standards.
Qualifications:
- RN License (required).
- Strong assessment and care planning skills.
- Excellent verbal and written communication skills to effectively collaborate with staff, patients, and their families.
- Ability to work independently with a flexible schedule.
Position Details:
- Supervisory Visits: Perform visits every 90 days.
- Pay Structure: Flat rate, per assessment visit. Each visit is 30 minutes to 1 hour in duration.
