This position functions as a non-clinical RN Case Manager in support of the Florida Army National Guard Office of the State Surgeon (OTSS). The principle duty of a non-clinical nurse case manager is to assist in reaching their goal of a full, medically/dentally-ready Unit Manning Roster (UMR) and upon alert, the Deployment Manning Document (DMD). They function as liaison between the unit of assignment and the Office of the State Surgeon. NCMs are required to be exceptional planners, coordinators, and facilitators. They are resourceful, identifying and implementing civilian and military resources/services in order to apply them to individual and group needs.
While Case Management continues to focus on catastrophic illness or injury, Case Management practices are also intensely directed at addressing chronic conditions that are more prevalent in the general patient population. Case managers can affect patient outcomes through proactive interventions across multiple healthcare settings. Additionally, case managers are expected to engage community resources and facilitate ongoing and consistent patient education.
The MHS has three primary goals for Case Management:
- Improve the care, management, and transition of recovering Service members.
- Broaden the application of CM to include those with complex and at-risk needs before the Soldier requires complex care.
- Continued utilization of medical case managers is essential to maintaining established medical readiness goals for deployability of the ARNG.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Apply current medical policy, render intake case management services at
PHA and SRP
Identify service member's individual medical needs, ensure that the DD Form
2766 is complete and accurate
Create Medically Non-Deployable (MND) referrals in E-Case to chart all medical correspondence
Collect all medical documentation necessary to substantiate the service member's deployment status
Ensure that any and all medical documentation is placed within the service member's individual medical record and scanned to their electronic record
Ensure that MEDPROS accurately reflects the service member's status
Generate provider review packets for clearing/profiling upon receipt of appropriate medical documentation
Ensure that the provider review packet is reviewed in a timely manner
Deliver provider's disposition to the unit of assignment and the appointed medical representative
Forward disposition and medical record to Health Systems Section (HSS) for board processing if applicable.
Provide up-to-date statistics regarding the medical readiness of their unit of assignment at meetings/IPRs
Coordinate with mobilization stations and include mob site specific requirements into the unit of assignment's get well plan
Incorporate DSS resources and services in order to augment the service member's treatment plan if applicable
Provide mentorship to appointed medical representatives within assigned case load
Work closely with the unit of assignment's command team and appointed medical representative.
Refers Soldiers to the appropriate agencies to obtain counseling and/or guidance on benefits provided by the Department of Labor, Social Security Administration, Wounded Warrior Program and Veterans
Counsels Soldiers on their rights, benefits, privileges or obligations and serves as
SME for the Integrated Disability Evaluation System (IDES) process.
Conduct Quality Assurance/Quality Control on medically non-deployable (MND) packets submitted to HSS and MTFs.
Performs other job related duties as required.
The Non-Clinical Case Manager works closely with and supervises assigned Administrative Care Coordinators who assist with obtaining and uploading required documentation.
EDUCATION and/or EXPERIENCE
The non-clinical case nurse manager requires the expertise as dictated by the 'practice of nursing" which is the systematic application of substantial specialized knowledge and skills derived from the biological, physical, and behavioral sciences, to the care, treatment, counsel, and health teaching of individuals who are experiencing changes in the normal health processes
or who require assistance in the maintenance of health and the prevention or management of illness, injury, or disability. Minimum qualifications are:
Active, unrestricted registered nurse license from any U.S. jurisdiction
Able to meet all licensure requirements in AR 40-68, Chapter 4-8
Active (unexpired) BLS certification by the American Heart Association
KNOWLEDGE, SKILLS AND ABILITIES
Strong organizational skills
The ability to use Power Point, Excel, Word, Access, Outlook
Operate a personal computer to input, store, retrieve and manipulate data for various reports as required by this PRS. Experience with computerized client management systems is required.
Use various software programs to maintain database files and prepare reports, including but not limited to the following ARNG web-based electronic data systems via MODS: Medically Non-Deployable (MND)/e-Profile, Medical Data Protection System (MEDPROS), MEDPROS Web Data Entry (MWDE), Soldier Patient Locator (SPL), Warrior Transition Report (WTR), DENCLASS, AVS, HRR, ePHA/MHA, eCase and other modules as applicable or as they become available.
Experience with the military, such as prior military service or military family member or military family volunteer, is highly desirable.
Demonstrates competency in assessing, planning, implementing, and evaluating care provided including patient/family education.
Demonstrates leadership qualities including professional verbal and written communication skills, flexibility to prioritize complex situations, decision-making skills, and professional development through participation in continuing education and professional organizations.
Combined Access Card (CAC) Requirements:
Before CAC issuance, the contractor employee requires, at a minimum, a favorably adjudicated National
Agency Check with Inquiries (NACI) or an equivalent or higher investigation in accordance with Army Directive 2014-05.
HSPD-12 Background Investigation Requirements:
The contractor ensures a CAC is obtained by all contract or subcontract personnel assigned to work on the Government site and by personnel requiring access to a DoD network are required to, at a minimum, have received a favorable FBI fingerprint check and must complete a National Agency Check with Inquiries (NACI) or equivalent/higher investigation which includes: OF306, one page Resume, PSIR Form, and fingerprint card.
Contractor employees shall have a favorable background investigation or hold a security clearance and access approvals commensurate with the level of information processed or available on the system. All contractor employees with access to a government info system must be registered in the ATCTS (Army Training Certification Tracking System) at commencement of services, and must successfully complete the DOD Information Assurance Awareness prior to access to the IS and then annually thereafter.
All contractor employees shall complete AT Level I awareness training within 10 calendar days of start date
This position is part of a team of non-clinical medical and behavioral health case managers and administrative care coordinators who work under the supervision of the Site Lead Care Coordinator and Program Manager to provide non-clinical case management services to the FLARNG to support CNHS contractual requirements.
Travel to other locations in Florida may be required in order to meet deliverable of this contract. All travel requirements (including purpose, plans, agenda, itinerary, or dates) shall be pre-approved by the government), and is on a strictly cost reimbursable basis.
