Oasis Home Healthcare, Inc is seeking Patient Care Coordinator/ Medical Assistant to join our team. Oasis Home Healthcare agency with a highly collaborative team which continues to grow and expand.


Patient Care Coordinator position is responsible to coordinate the daily processing of referral-to-admission for home health services. The Care Coordinator manages all aspects of the patient intake process in a timely manner including but not limited to assigning patients to appropriate clinicians, verifying insurance coverage and benefits, contacting referral sources and informing admission status, and obtaining the necessary documentation.

  • Receive and analyze incoming referral and order calls.
  • Verify insurance information.
  • Educate prospective patients, families, physicians, and hospitals regarding home health services available.

Greets patients in a service oriented and professional manner, answers phones, send faxes, follow-up with MD offices for orders.

At least two years of relevant medical office experience is required to be considered for this position and strong job tenure. No job hoppers (do not apply if you switch positions every year, you will not be considered).

  • Two years of medical experience
  • Clean Background Check
  • Good communication skills (Both in writing and verbal commands)
  • Professional attire


  • High school or GED required; associate degree preferred.
  • Excellent verbal and communication skills.
  • Knowledge of insurance policies for home care authorizations.
  • Detail-oriented, self-starter and able to work independently
  • Ability to multi-task and prioritize
  • Working knowledge of medical terminology
  • Excellent computer skills and working knowledge of Microsoft Office (Word and Excel), copier, fax machine, and printers.
  • Client focus both internal and external.
  • Problem-solving and ability to make a sound decision.
  • Productivity: prioritization, planning, and organizing.
  • Acceptance of philosophy and goals of this Agency.
  • Ability to exercise initiative and independent judgment.
