The Department of Family Medicine & Community Health at UMass Memorial Health Care is recruiting a full time or part time Primary Care Physician for Community Healthlink, Inc. a community- based mental health center with locations in both Leominster and Worcester, Massachusetts. Community Healthlink’s Homeless Outreach and Advocacy Project (HOAP) has provided integrated primary and behavioral health care and substance use treatment for more than 30 years. Our highly-qualified, trauma-informed providers and clinicians utilize case managers, care consultants, and nurse care managers to provide a person-centered team approach to meet the needs of each patient through individual and group treatment. The HOAP Clinic is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) and is a recognized Patient-Centered Medical Home (PCMH) practice.
Ideal candidates will be BE/BC in Family Medicine or Internal Medicine with a passion for serving a vulnerable patient population. Candidates must have an interest in doing minor procedures and women’s health. We seek candidates with experience and interest in community outreach. Bilingual Spanish language skills are desirable.
