Job Summary:
A member of patient care team within the Department of Hospice and directly accountable to Hospice Director or Patient Care Manager. Indirect supervision of Program Coordinator. The position requires that the individual assist the professional nursing staff by performing specific nurse aide functions to include bathing, shampooing, ambulation, transfers, tidy sick area and other duties as identified on care plan according to the patient’s IDT care plan related to patient care under the supervision of a registered nurse or therapist. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. The CNA prepares documentation of patient care in the home at the time of visit as applicable and submits documentation in a timely manner according to policy. Notifies RN of any changes in patient and assists in meeting needs. CNA participates in inservice and competency programs as applicable. Performs other job duties as assigned.
Performance Expectations:
- Demonstrates ability to bathe patient in shower or tub using appropriate water temperature, preventing falls, and maintain privacy.
- Demonstrates ability to transfers patients safely utilizing safe body mechanics for yourself and patient.
- Demonstrates ability to document in the EMR according to policies and procedures.
Qualifications:
Education/Skills
High School Diploma/GED Required. Completion of an approved Nurse Aide training course required.
During department orientation, required to successfully complete a 75 hour certification program as a
Home Health aide. Completion of a Nurse Aide training program from an approved school preferred. If
no prior certificate must successfully complete a 75 hour certification program as a Hospice aide.
Work Experience:
Six months of nurse aide work experience preferred.
Certification/Licensure-DUE UPON HIRE
- Certified Nurse Aid Certificate-preferred
- Valid Driver's License with proof of Automobile Insurance.
Additional Certification/Licensure - obtained based on the required timeframe below
