Full Job Description
Job Summary Responsible for providing direct services to patients and families to identify and resolve problems that impede discharge planning or the patient's use of health care resources. This position involves contact with patient, families, physicians, nursing, ancillary staff, and internal/external resources. This position requires documentation of all interventions and services provided. Requirements Bachelor's Degree in Social Work from an accredited School of Social Work required, Master's degree preferred; Current licensure as a Social Worker in compliance with Maryland's Board of Social Work preferred but must obtain within 12 months of hire; One year experience in a health care setting; hospital preferred.
