Day Habilitations Specialist (DSP) Full time Queens
Must have a valid driver's license for full time day hab position
The following are the duties and responsibilities for this position:
1. Transport clients to and from Program on a daily basis.
2. Assist clients with ADL’s
3. Have sufficient knowledge to fulfill their role as it pertains to service plan.
4. Promote health and wellness of clients through health maintenance and prevention strategies, knowledge of general health and safety rules, first aid and emergency procedures.
5. Know organizational mission, policies and procedures, understand their role and responsibilities within the organization, and respect the organization in a responsible manner.
6. Advocate for clients as mandated reporters of abuse, neglect and maltreatment.
7. Be aware of the requirement for documentation within the program and organization as a whole and manage those requirements in a thorough and efficient manner.
8. Enhance the ability of the client to lead a self-determined life providing the support and information necessary to build self-esteem, assertiveness and independence.
9. Knowledge of formal and informal resources and supports available in their community and are skilled in assisting the clients to gain access to such supports.
10. Assist clients in building/strengthening skills necessary to develop and maintain friendships and other personal relationships.
11. Utilize the range of effective communication strategies and skills necessary to establish collaborative relationship with clients, families and resources.
12. Perform job duties and interact with clients and co-workers in a professional manner at all times.
13. Report to other locations when assigned.
14. Provide coverage in accordance with the staffing schedule established by either the Department Director or other supervisor with the understanding that changes in schedules and program assignments may be necessary in order to meet the needs of the clients and program.
15. Attend and successfully complete all required trainings.
16. Perform such other duties as assigned.
17. Adhere to agency and program policies, procedures, conduct and rules including Agency Compliance Policies and Procedures.
18. Participate in screenings of potential patients (on-site and off-site)
19. Stay beyond working hours in necessary situation.
20. Participate in special assignments or projects as dictated by the need of the program.
21. Maintain confidentiality of all patients; consult with Supervisor prior to releasing confidential information; obtain release of information from patient prior to any disclosures of patient information.
QUALIFICATION
Valid NYS Driver License. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE
GED, High School Diploma or B.A. from an accredited school. Ability to read and interpret documents such as government regulations and procedure manuals. Ability to speak effectively before groups of consumers or employees of organizations
