Legacy Medical Care (LMC) is looking for a Licensed Clinician Social Worker (LCSW) will provide services to individual patients (including children, adolescents, and adults) and family behavioral counseling to the patients of Legacy Medical Care. The LCSW will engage in behavioral assessments to identify emotional, social, and environmental strengths and problems related to their, illness, treatment, and or life situation.
TASKS AND DUTIES:
· Treat problems within the scope of competence and exercise judgment on problems requiring consultation, referral, or evaluation
· Counsel patients on medical problems, use of medications, expected effects of treatments, diet, and other health maintenance matters.
· Manage behavioral or psychosocial problems including crisis intervention and short-term individual and family counseling
· Analyze and interpret data, formulate diagnoses and problem lists, and establish plans for the management of health problems
· Initiate consultation requests to specialists and other health professionals including physical therapists, occupational therapists and dietitians
· Completes required documentation and paperwork including behavioral assessments and treatment planning and implementation in a timely manner
· Monitors and coordinates the delivery of health services for patients as related to behavioral care, including linking with other treatment providers not only within the primary care setting but, with the patient’s permission, outside it as well
· Provides patients with self-management skills and educational information needed so they can be full participants in their own treatment
· Provide and communicate recommendations for interventions
· Other duties as assigned
RESPONSIBILITIES:
· Prepare and maintain complex treatment and progress reports and document patient medical charts as needed which include diagnostic and treatment recommendations, discharge planning and follow-up services
· Work collaboratively with a wide range of clinical and non-clinical disciplines to assess patient and family needs, design effective care planning or service goals or medical treatment plans regarding utilization of additional resources
· Management of teen crisis
· Responsible to assure the integrity of the system meets regulatory requirements, reviews high-risk areas and is always in a survey ready state
· Maintains weekly scheduled appointments in our EHR system
· Maintain all CME (continuing medical education) required
KNOWLEDGE:
· Knowledge of administrative and clerical procedures and systems such as word processing systems, filing and records management systems, forms, and other office procedures
· Knowledge of computer hardware and software, including applications and clinical programming
· Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar
· Knowledge of principles and processes for providing customer and personal services including needs assessment techniques, quality service standards, and customer satisfaction evaluation techniques

· Ability to deal with nonverbal symbolism (formulas, scientific equations, graphs, etc.,) in its most difficult phases.

Qualifications
- Must hold a valid license as a Clinical Social Worker
- Proficiency in behavioral therapy, patient care, and medical documentation
- Experience in patient assessment and clinical counseling
- Knowledge of HIPAA regulations and medical terminology
- Familiarity with EMR systems and medical records management
- Ability to provide crisis intervention and work with individuals with developmental disabilities
Job Type: Full-time
Pay: $65,000.00 - $75,000.00 per year
