American CareQuest is actively looking for Hospice Clinical Supervisor (RN) to lead our Clinical Team!
Job Type: Full-time + Benefits + $5,000 SIGN-ON BONUSES

Qualifications:
- Current California RN License
- Current CPR/ BLS Certification
- Valid Driver's License
- 1 Year: RN experience
Responsibilities:
- Review case referrals and assign appropriate clinicians to each case
- Collaborate with hospice staff to provide effective coordination of services and evaluate delivery of care
- Assure patient's needs are continually assessed through development, implementation, and updates of individualized plans of care
- Conduct quarterly record reviews and communicate findings and recommendations to upper management
- Oversees staffing and visit assignments. Supervisory visits and training in the field
- Tracking and reporting of clinical data
- Conducts weekly case conferences
- Assures that a reassessment of a patient's needs is performed by the appropriate health care professional when there is a significant change in the patient's condition, at the physician's request, or after hospital discharge

Pay: $130,000.00 - $200,000.00 per year
